Help Us Update Our Mailing List

The Resource Center is updating its mailing list. If you receive our Closing the GAP or HIV Impact newsletters, you are on our list. To verify your address, correct errors, or give us a new address, you will be taken through a series of three steps. Please follow the link below to our new mailing list PROCESS form.

STEP 1: On the PROCESS form, you will be asked to enter the account number and zip code which are located on the mailing label of your most recent newsletter. Once you type in these numbers and click the PROCESS button, you will be directed to an UPDATE form.

STEP 2: The UPDATE form consists of two parts. Part I displays your current information, as it appears in our database. If all of the displayed information is correct, please enter the word "VERIFIED" in the comments field on the UPDATE form which will be at the bottom of the page, then hit the UPDATE button. If the information is not correct, proceed to Part II.

In Part II of the UPDATE form you may make changes, add comments, or request to remove your name from the mailing list. Whether you are updating or confirming your record, please include your e-mail address. If you have no email, just type "NO EMAIL" in the email field of the form. Please enter only the information in the form that requires correction, then hit the UPDATE button.

STEP 3: Clicking on the UPDATE button will take you to the next step in the process, the SUBMIT page. The SUBMIT page will show you the corrections, deletions or omissions you intend to submit to OMHRC. If you made a mistake, use your browser's BACK button to return to the UPDATE page to make the correction. Hit the UPDATE button again, and you will be returned to the SUBMIT page. Once you are satisfied with your changes on the SUBMIT page, hit the SUBMIT button. Your request will be sent to OMHRC.

NOTE: OMHRC has established an e-mail list that we will use to send out an electronic-alert regarding updates to our web site, services, and other minority health information. It will contain a variety of links to minority health news, featured health articles, and specialty Web pages from our site. If we do not have your email address and you would like to receive our monthly "E-alerts", please include your email address in the update form.

If you do not wish to receive E-alerts, please mark NO on your update form. Unless otherwise indicated, you will continue to receive our CTG and HIV Impact newsletters. We will not share this list with any commercial, or non-profit organization. In addition, only the OMHRC staff will send monthly alerts to you. List members can not send broadcast messages to the rest of the names on our mailing list.
