Privacy Act Statement
AUTHORITY: 10 U.S.C. 8012 PRINCIPAL PURPOSE: To obtain information for computer input from student voluntarily enrolled in the AWC Nonresident Studies Program. ROUTINE USES: Used to prepare data for input into the AWC database. This program enrolls, reenrolls, cancels, and graduates AWC Nonresident Studies students. This information creates the database against which student writing assignments, exam scores and course completion can be correlated. Not releasable outside DoD. DISCLOSURES: Voluntary. However, if requested information is not furnished, enrollment cannot be accomplished.
INSTRUCTIONS
Fill out this form completely, print a copy, sign it and take it to your Education Officer who will add the 10-digit TCF number and mail (AWC/NSE, 325 Chennault Circle, Maxwell AFB, AL 36112-6427) it to us. International applicants do not use this form to enroll in the nonresident program. International applicants who meet the eligibilty criteria should follow these instructions.
