2004 Premium Instructions and Forms

New for 2004!

You can now submit filings and payments for the 2004 plan year and later online. Learn more about My Plan Administration Account (My PAA), PBGC's new electronic premium filing option for pension plan practitioners.

Note that, effective with the 2004 plan year (January 1, 2004), Bank One, NA, of Chicago will process all premium payments and will receive all delivery services, wire transfers, and premium-related correspondence. Specific address information can be found on page two of the 2004 Estimated Premium Instructions or page ii of the 2004 Instructions.
