The following ATF public-use forms may be downloaded for printing. At present, these forms MUST be submitted in hard-copy. All forms are in Adobe PDF format. You may download Adobe Acrobat Reader by using the link at the bottom of this page.

Distribution Center Order FormDistribution Center Order Form - This Online form can be used to order the most frequently requested items from the ATF Distribution Center. This list will be updated periodically to include additional items. For items not listed, please call (703) 455-7801 to order.

Instructions

The current ATF Form # comes first. The old ATF Form #, if available, is in parenthesis. The GILS# refers to the form's record in the Government Information Locator service at GPO Access.

You can order forms, publications and other informational items online at the Distribution Center Order Form.

You may comment on any ATF form by writing to the Document Services Branch, ATF, 650 Massachusetts Avenue, NW., Washington, DC 20226, or by sending your comments by e-mail to ATF. Also, you can examine the Federal Register notices, as required by the Paperwork Reduction Act of 1995.
