General Statement

Background

In FY 1996, the House Appropriations Committee instructed the Department of Health and Human Services to:

...develop and submit to the Committee, as part of the President’s budget request each year, a comprehensive research, demonstration, and evaluation budget and plan that indicates Departmental programmatic goals to be achieved through its research, demonstration, and evaluation activities and how each separate program or activity supports these overall goals.

In FY 1997, the House Appropriations Committee report accompanying H.R. 3610 commended the Department on its prior year report and budget plan and instructed the Department to:

...continue to submit this document to the Committee as part of the President’s budget request each year.

The committee also advised the Department to:

...expand its definition of activities covered by the report so that all health services research and related prevention research and demonstration activities of the National Institutes of Health are included.

The following FY 2003 budget/plan is submitted by the Department in response to these requests.

Scope

This budget includes research, demonstration, and evaluation activities authorized under the jurisdiction of the House Appropriations Committee, except for basic and applied medical research. Also not included in the budget are data collection activities (surveys) supported by the Center for Medicare and Medicaid Services (CMS), the National Center for Health Statistics (NCHS), the Agency for Healthcare Research and Quality (AHRQ), and the Substance Abuse and Mental Health Services Administration (SAMHSA), including data collection activities funded through congressional earmarks of the Public Health Service evaluation funds authorized by Section 241(a) of the Public Health Service Act. Section 241(a) funds for use in conducting evaluations of Public Health Service programs and for support of research at AHRQ are included in the budget.

FY 2003 Request

The Department’s research, demonstration, and evaluation budget (RD&E) for FY 2003 totals $1,977,339,000. HHS RD&E activities included in the budget will directly support the accomplishment of a number of goals and objectives in the Department’s FY 2001-2006 Strategic Plan. The three-year updated plan was submitted to Congress on September 30, 2000. The table that follows summarizes the RD&E budget requests by objective in the Department Strategic Plan.

Budget Presentation

Some Department RD&E activities address more than one goal or objective. Cross-references are provided where relevant. In cases where it is practical to separate an activity into discrete components, the activity is displayed under the various goals or objectives that the activity addresses. In these cases, the estimated budget represents only that portion of the RD&E activity attributable to the goal or objective under which the activity is displayed.

In other cases, Department RD&E activities cannot be easily separated into components that address specific strategic objectives because the purpose of the activity is cross-cutting. In these cases, the information is displayed beginning with Table XXXIV - Crosscutting Research, Demonstration, and Evaluation Activities. Specifically, the Office of Inspector General (OIG), the Office of Assistant Secretary for Planning and Evaluation (OASPE), and the Office of Public Health and Science (OPHS) conduct evaluations (and inspections in the case of OIG) that often involve multiple strategic objectives. For example, OASPE evaluations of state welfare reform are relevant to Objectives 2.1 on economic independence, 2.2 on child support, and 2.4 on security of children and youth. OASPE’s disability, aging, and long-term care policy research generally touch on Objectives 2.5 on older Americans, 2.6 on long-term care, 3.4 on Medicare beneficiaries, and 3.5 on the fiscal integrity of CMS programs. OPHS performs evaluations that often affect several Public Health Service agencies, such as its FY 2003 plan to evaluate the Leading Health Indicators of Healthy People 2010.

Planning Coordination

Responsibility for coordinating the development of the HHS research, demonstration, and evaluation agenda resides with the Assistant Secretary for Planning and Evaluation. A standing committee of planning and evaluation officers from across the Department provides advice on coordination issues and helps to integrate the various research, demonstration, and evaluation activities across the Department.
