Corrective Action: Babyhood will replace the seat cushion with a cushion that meets the compression/deflection requirements.

Noncompliance:  These child safety seats do not meet the impact release requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems." The buckle could require excessive hand pressure to release and could hamper removal of the child in an emergency situation.
Corrective Action:  Babyhood will provide a modified buckle tip and replacement instructions directly to owners of these seats.

Model(s): Galaxy 2000 Booster Seat Model 960 and 961 (with back).
Production Dates: November 25, 1998, to January 31, 1999

Noncompliance: These seats were improperly manufactured with defective backs and cushions which are weaker in strength. In the event of a vehicle crash, the child seat backs and cushions can completely separate. These seats do not meet the requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Consumers will be sent free replacement parts and installation instructions.

Model(s): Galaxy 2000 Booster Seat Model 960 and 961 (with back).
Production Dates: All Galaxy 2000 Booster Seats are included in the recall.
Noncompliance: When the shoulder belt portion of a vehicle's 3-point safety harness is not properly positioned, excessive chest forces may result during a sudden deceleration. These seats do not meet the requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Instructions will be provided to show consumers the proper position for the seat belt when using the Galaxy 2000.

BRITAX CHILD SAFETY, INC.   1-888-4BRITAX (888-427-4829).

 

Model(s): All Marathon (E9L06), Wizard (E9L07), Husky (E9L30) child restraints, as well as restraints manufactured by Britax for Snug Seat under the model name Traveller Plus (E9L32).
Production Dates:

  • Marathon - 8/2/02 - 7/23/04
  • Husky - 8/11/02 - 7/23/04
  • Wizard - 7/16/03 - 7/23/04
  • Traveller Plus - 12/2/03 - 7/23/04
Defect: The A-Lok adjuster assembly can allow slippage of the webbing resulting in loosening of the harness straps. If the slippage were to occur in the event of a vehicle crash, the child would move too far forward, possibly increasing the risk of injury.
Corrective Action: Registered owners will receive a replacement adjuster strap by September 30, 2004. In the meantime, registered owners will receive instructions on how to check the seat for slippage after every use. If slippage is occurring before replacement straps can be mailed, the use of the restraint should be stopped and customers should contact Britax for further assistance. Non-registered owners may obtain the free remedy by calling Britax at 1-800-683-2045, or visit Britax's web site, www.BritaxUSA.com, to receive a free kit or get more information. Non-registered owners are strongly urged to register their child seat with the manufacturer in case of future safety recalls.

Production Dates: October 7, 2002 to December 8, 2003
Noncompliance: In the event of a vehicle crash, these restraints, when secured with a lap belt as described in the user's manual, could allow the child's head to move further forward than allowed by Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Registered owners will be provided with labels and instructions for the application of the labels to be placed over the corresponding instructions in the current user's guide and on the restraint cover. Non-registered owners are strongly urged to register their child seat with the manufacturer in case of future safety recalls. Parents should continue to use the seat while waiting to receive and install the repair kit because it will still provide better protection than an adult seat belt or no restraint.

Production Dates: April 25, 2001, to February 15, 2002
Noncompliance: These seats do not comply with Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems." In the event of a vehicle crash, forces could cause the child's head to move further forward than allowed by Safety Standard 213.
Corrective Action: All consumers who returned a registration card to Britax will automatically receive a free replacement harness system along with instructions - including TPE pads (also known as hugs - harness ultra-guard system).

Production Dates: March 5, 2001, to July 26, 2001
Noncompliance: The tether brackets used during the affected period may allow one of the tether attachments to separate from the seat shell in a vehicle crash. Separation of one of the tether brackets from the seat shell could cause increased head excursion. These seats do not comply with Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Britax will send out a metal ring replacement kit along with repair instructions to its customers free of charge. Customers can contact Britax at 704-409-1700 prompt 2 for questions and issues and to request the free replacement kit.

Noncompliance: The shells manufactured during this time may have been improperly molded and could allow higher head excursions than allowed by Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems." In a crash, additional head movement could allow a child's head to strike something in the vehicle and lead to injury. Customers are being instructed to call 1-800-683-2045 for this recall campaign only.

[Britax Safety Recall No. 060001]

 

Model(s): Freeway Model 101
Production Dates: June 1, 1996 - April 18, 1997
Noncompliance: The belt buckle can open by lifting up on the "pushbutton" at less than the 9 pound minimum force required in Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems." This means that a child can more easily open the buckle.
Corrective Action: Britax will provide owners with a newly designed replacement buckle and installation instructions.

CENTURY (GRACO CHILDREN'S PRODUCTS)   1-800-837-4044

Models: Encore (44612 GTn and 4662 WSC 01); Bravo (44622 LTG, 44630 LTH, 44631 PFN, and 44662 WSC); Accel (45100 CHT, 45100 MTE, 45100 STH, 45200 LFT, 45200 MXM, 45600 JTI, 45600 MTE, 45600 MTO, 45600 MXM, 45600 SAF, 45700 GSD, and 45700 LFT); and Smartmove (44709 MTE and 44709 SVR).

Production Dates: seats shipped between April 9 and May 10, 2001
Defect: These seats are equipped with a central adjuster (A-Lok Adjuster) device which the consumer tightens the child seat harness straps on the child. In the event of a vehicle crash the webbing could slip 4 to 6 inches which would not adequately protect the seat occupant, possibly resulting in injury or death.
Corrective Action: Century will notify its customers and replace the seat free of charge. Customers can contact Century Costumer Service at 1-800-837-4044 for a replacement seat or more information.

Models: All rear-facing infant car seats/carriers - (Graco Children's Products, Inc.)
Production Dates: January 1, 1991, - July 31, 1997
Defect: The handle can crack during normal use which can led to an unexpected break. If the handle breaks, the child could fall to the ground possibly causing serious injuries.
Corrective Action: To prevent future cracking or failure of the child seat handle, Century will provide owners with an improved-design handle and installation instructions.

Model(s): Century Breverra Premiere and Contour Booster Car Seats
Production Dates: February 1995 - July 10, 1998
Defect: This is not a safety recall in accordance with Federal Regulation Part 573. However, it is deemed a product improvement campaign by the agency. The buckle for the harness system on these booster seats can crack or break.
Corrective Action: Century has decided to conduct a product improvement campaign to replace the buckle assembly. Century will provide a free replacement buckle kit and installation instructions.

Production Dates: September 12, 1995 - May 13, 1996
Defect: Due to mis-assembly of the buckle assemblies, the buckles can release upon impact.
Corrective Action: Century will provide a free replacement buckle kit and installation instructions.

Production Dates: November 1995 - February 1996
Noncompliance: This child safety seat utilizes a latch assembly where a variation in the components resulted in the potential for a buckle to release. This does not conform to the dynamic testing requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Century will provide a retrofit kit so owners can easily replace the latch assembly.

Production Dates: December 1991 - April 1992
Noncompliance: The red release button can rotate upward in a crash or sudden stop, making it possible for the buckle to release the shoulder harness.
Corrective Action: Century will provide a kit to retrofit the child safety seat with a modified release button assembly to consumers.

Production Dates: September 1989 - April 1992
Defect: Some red buttons shipped to repair buckle release buttons recalled under NHTSA Campaign No. 92C-017 were incorrect. The wrong red release button would not allow the strap to engage the buckle correctly, causing a false latch situation.
Corrective Action: Century has issued graphic instructions to all seat owners who may have received the wrong part. The instructions provide details how to determine whether the previously shipped red release button is correct. If the wrong button was shipped and the owner notified Century of the problem, Century will ship the owner the correct button.

Noncompliance: The red shoulder strap release button can rotate out of the frame in a crash or sudden stop. In this event, the release button could not be activated with less than 16 pounds of force as required by Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Century will provide a kit to retrofit the child safety seat with a modified release button and dress plate to consumers.

Defect: The directions for threading the shoulder straps through the metal retention slides are unclear.
Corrective Action: Century will provide owners with new instructions and a descriptive label so they can re-thread the shoulder straps correctly.

Production Dates: March 1987 - December 1990
Noncompliance: Buckle release force can exceed 16 pounds when not used in appropriate forward latch position for a child weighing 30 pounds or more as required by Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Century will issue notification, change instructions and labeling to indicate that the rear latch position is not to be used with children weighing 30 pounds or more.

Model(s): 3000STE Models 4365MC, 4365DT, 4366BH, 4368BH and 5000STE Models 4450GD, 4460DT, 4460FE, 4470MC, and 4475GU.
Production Dates: May - June 1990
Noncompliance: Latch tongues broke during impact in routine infant car seat crash testing at 30mph as required by Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems." During sever impact, the child might submarine out of the seat with potential for sever injury or death.
Corrective Action: Century will replace the latch tongue.

Model(s): 4450GD, 4475GA, and 4460FE
Production Dates: December 1988 - November 1989
Noncompliance: These safety seats exceed the post impact buckle release force requirement of 16 pounds maximum during Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems" testing. In a traffic accident, this could prevent the adult or parent from quickly removing the child from the car seat.
Corrective Action: Century will replace the harness assemblies with a new version that includes a modified latch tongue.

Production Dates: June 1986 - October 1989
Defect: The retaining screws on the handle to the shell can loosen, allowing the handle to separate from the shell.
Corrective Action: Century will provide owners with newly designed screws and installation instructions.

DOREL JUVENILE GROUP (Previously COSCO)    800-880-9435

 

Model(s): High Rise child restraints, Models 22-297, 22-298, 22-299, and 93-299
Production Dates: October 1, 2003, through October 31, 2003
Defect: With an affixed label on the seat, DJG intended to recommend that the forward-facing mode should be used for children "over 1 year of age." However, the label was misprinted to recommend that the forward-facing mode be used for children "under 1 year of age." If the consumer were to follow the erroneous instructions, the seat may not adequately restrain the child in the event of a vehicle crash, possibly resulting in injuries to the child.
Corrective Action: Dorel Juvenile Group will mail a corrected label along with repair instructions to registered owners free of charge. Customers who have not registered their child seat should contact DJG toll-free at (866) 611-3022 to obtain the free repair kit.

Model(s): Alpha Omega and Eddie Bauer 3-in-1 child restraints, Models 02-331, 02-332, 02-531, 02-533, 02-537, and 02-555
Production Dates: November 17, 2000, and January 10, 2001
Noncompliance: The tether webbing could be cut and separate in a crash, which constitutes a failure to meet the requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems." In the event of a crash, the child may not be properly restrained, increasing the risk of injury.
Corrective Action: Dorel Juvenile Group will mail a free in-home repair kit along with repair instructions to registered owners. Nonregistered owners of these seats are advised to check the manufacturing date and model number to determine if their seat is affected by the recall. If so, owners should contact Dorel by visiting their website at www.coscojuvenile.com, click on "Safety Notices" or call Dorel's toll-free number (866-611-3022) to receive a free repair kit. Until the repair kit is installed, owners are instructed to continue to use the seat as directed.

Defect: When used as an infant carrier, the handle locks on each side of the seat can unexpectedly release, causing the seat and infant to flip forward which could result in serious injuries.
Corrective Action: Dorel Juvenile Group will notify registered owners about this recall. All owners are advised to check the manufacturing date and model number to determine if their seat is affected by the recall. If so, owners should contact Dorel by visiting their website at www.djgusa.com, click on "consumer services" or calling Dorel's toll-free number (800-880-9435) to receive a free repair kit. Until the repair kit is installed, owners are instructed not to carry the seat by its handle.

Models: Safety 1st and Beatrix Potter Designer 22, Model Nos. 02-621 SAL, 02-620 AZY, 02-620 BEA
Production Dates: January 3 through February 13, 2002.
Defect: When used as a carrier, the handle may slip from one position to another, causing the seat to fall forward. If the child is not properly buckled into the seat, the child can fall out if the carrier, possibly resulting in injuries.
Corrective Action: Dorel Juvenile Group will notify its customers and provide a repair kit along with installation instruction free of charge. Owners who do not receive the repair kit within a reasonable time should contact DJG at 800-536-1090.
NHTSA Recall No.:  02C027000 (PREVIOUS RECALL NUMBER 02E-027)

 

Models: Arriva -02-729-TED infant child restraints.
Production Dates: March 31 through April 7, 1999
Noncompliance: Some of these units were produced without an air bag warning label. This does not comply with the requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Consumers will be mail an air bag warning label and placement instructions.

Noncompliance: These seats fail to fully comply with the requirements of Federal Motor Vehicle Safety Standard No. 213, “Child Restraint Systems.” The buckle release force exceeds the regulation.
Corrective Action: Cosco will send a remedy kit consisting of a new buckle assembly, the screws required, and detailed installation instructions.

Models: Arriva - 02-665, 02-732, 02-756, 02-729, 02-733, 02-757, 02-731, and 02-751; and Turnabout - 02-667, 02-761, 02-764, 02-758, 02-759, 02-762, 02-765, 02-760, and 02-763.
Production Dates: March 1, 1995 through September 9, 1997
Defect: When used as an infant carrier, the handle locks on each side of the seat can unexpectedly release, causing the seat to flip forward. When this happens, the infant could fall to the ground and suffer serious injuries.
Corrective Action: Cosco will provide the owners with a free repair kit which will make the handle stronger. Owners are also being advised not to carry the seat by the handle until the repair kit has been installed.

Model(s): Touriva 5-Point and Touriva Overhead Shield
Production Dates: August 9 - August 22, 1997
Noncompliance: When these seats were tested dynamically per the requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems," structural failure occurred in the belt path area of the shell in the forward facing reclined position only.
Corrective Action: Cosco will provide inspection instructions to the owner. If the seat is involved in the campaign, a remedy kit will be provided to the owner.

Model(s): Arriva 02-741
Production Dates: March 1 - May 31, 1995
Defect: Separation of the buckle latch release button and spring from the latch release housing. The seat will become inoperative as the user will not be able to reinsert the spring and red latch release button properly into the housing.
Corrective Action: Cosco will replace the entire latch release assembly.

Model(s): Dreamride Ultra
Production Dates: April 1994 - June 1995
Noncompliance: These seats do not meet the maximum back angle requirements of Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action: Cosco will provide a retrofit kit to owners of these seats which will include an easy-to-install metal plate that hooks over the base of the seat and is securely snapped in place by two plastic fasteners.

Model(s): 02-034 and 02-404
Production Dates: May - August 1994
Defect: In a vehicle crash, at speeds greater that 30 miles per hour, the buckle latch housing mounted in the base of the shell can deform, causing the latch mechanism to fracture, allowing disengagement of the buckle tongue from the buckle housing.
Corrective Action: Cosco will send to owners a replacement buckle housing which can be easily replaced by removing the four screws attaching the original buckle latch assembly.

Model(s): TLC infant car seat, 01-188-ARQ
Production Dates: April - December 1993
Noncompliance: The ribbon quilt child seat cover has been tested and the material burns faster than the rate established for child seat material. The child seats do not comply with Federal Motor Vehicle Safety Standard No. 302, "Flammability of Interior Materials."
Corrective Action: Cosco will send owners a replacement child safety seat cover.

Noncompliance: The fabric cover of the pads on these seats do not meet the requirements of Federal Motor Vehicle Safety Standard No. 302, "Flammability of Interior Materials."
Corrective Action: Cosco will provide a replacement pad for these child seats.

Affected Seats: The recall affects the "Convenience Base" of the seat only. Model numbers of the affected seats begin with 200 and 211; or 447 and 499 when sold with a stroller; and 639 on the bases that were sold separately.
Defect: Evenflo has discovered that after repeated use, two alignment posts in the base can become bent. If the posts become bent, consumers may have difficulty installing the carrier onto the base. Bent posts could also interfere with proper latching of the carrier to the base, preventing the child from being properly restrained.
Corrective Action: Evenflo will provide a free repair kit that prevents the posts in the base from bending. The kit includes instructions and metal clips that slip over the alignment posts. The clips are easy to install and it only takes a few minutes.

Corrective Action:  Evenflo intends to notify consumers of the potential problem and to provide consumers with a new replacement seat. Consumers will be provided a pre-paid return shipping label and will be asked to return their original seat in the box the replacement seat came in. Evenflo will notify consumers to stop using the seat's internal 5-point harness system, and will advise that the seat can only be used as a belt positioning booster seat.

Defect: If the Mercedes-Benz Infant Restraint or the Booster Seat is incorrectly positioned or moves several inches or more forward of the seat back in the front passenger seat of the Mercedes-Benz BabySmart™ equipped vehicles, it may not automatically deactivate the passenger front airbag. The status of the BabySmart™ system in the vehicle will, however, always correctly indicate to the driver if the airbag is deactivated or not, by the illumination or non-illumination of the "AIRBAG OFF" indicator.
Corrective Action: Mercedes-Benz will contact owners and instruct them to bring the recall restraints/seats to a Mercedes-Benz retailer for a replacement seat free of charge. A video illustration of the correct installation procedure will also be provided with every replacement seat.

Noncompliance: The seats have a snap in the upholstery directly over the locking belt adjustment mechanism (A-Loc Adjuster). When the upholstery is attached by the snap to the cover of the A-Loc Adjuster, it may cause tension in the upholstery due to the weight of the child, which may prevent the complete engagement of the A-Loc Adjuster allowing some slippage in the harness restraints. These seats fail to comply with Federal Motor Vehicle Safety Standard No. 213 "Child Restraint Systems." In the event of a vehicle crash, the child may not be properly restrained.
Corrective Action: Peg Perego will notify its customers and send repair instructions free of charge. Within 4-6 weeks, owners will receive a new plastic cover. Customers can register for instructions and a replacement cover by visiting Peg Perego's website at www.perego.com.

Noncompliance: These seats do not comply with Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems." In the event of a vehicle crash, forces could cause the child's head to move further forward than allowed by Safety Standard 213, increasing the risk of head injury to the child.
Corrective Action: Safeline Kids will mail repair instructions and a revised owners manual. The customers are being asked to remove and discard the shoulder and buckle pads from the harness before continuing to use the seat. The shoulder and buckle pads may interfere with the harness becoming properly snug. Also, customers should only use the upper forward-facing belt path. There is no problem when the seat is used in the rear-facing position for children weighing less than 20 pounds.

Corrective Action: XSCi has notified its customers and is offering a replacement seat (produced by an alternate child seat manufacturer) and will pay for the return of the noncompliant seat. XSCi will repair the noncompliant seat and return it to the customer. Owners who do not receive the free seat replacement within a reasonable time should contact XSCi at 800-630-6850.

Defect:  Mini vans equipped with integrated child seats. The shoulder harness restraint on the integrated child seat can become difficult to extract when the safety belt latch plate becomes contaminated. This can cause difficulty in releasing a child from the restraint.
Corrective Action:  Dealers will clean the latch mechanism. An extender will be added to the emergency release anchor for the child safety seat belts so that the release clips are more visible and accessible to the operator in the event that the retractor mechanism cannot be unlocked. In addition, an instructional owner's manual supplement card will be provided.

Defect: Second row bench seats with optional integrated child seats (ICS). The headrest lockout mechanism on the seats may not engage and lock the headrest in the "down" position, as intended, when the ICS is deployed. A vehicle occupant could move the headrest to the "up" position. As a result of the headrest being in the "up" position, a child seated in the ICS would receive less than the intended protection in the child headrest area in the event of a crash.
Corrective Action: The second row bench seats have been replaced with seats built with revised headrest lockout mechanisms.
Notes: All owners have been notified and the seats have been replaced in the affected vehicles.

Production Dates:  May 1996 to September 1997
Noncompliance:  Some of these vehicles may be missing a seat belt retractor clutch spring and/or pawl spring in the child seat. If the clutch spring were missing from the retractor, the seat belt would lock in the retract position and could not be used. If the pawl were missing from the retractor, the seat belt restraint would continually play out and would not lock. This could increase the occupant's risk of injury in the event of a vehicle crash. These seats fail to conform to Federal Motor Vehicle Safety Standard No. 213, "Child Restraint Systems."
Corrective Action:  To prevent the possibility or this condition occurring, dealers are to inspect the seat belts on the child seat, and if necessary, replace the child seat.
