Instructions:

    Purpose of Form: To petition for an alien worker to become a permanent resident in the United States. This form is filed on behalf of an alien, and not by the alien.
    Number of Pages: 7
    Edition Date: 4/16/04. 9/26/03 version may be used.
    Where to File:

    File this form with the Service Center with jurisdiction over the state where the alien will be employed. Use the addresses listed below when mailing Form I-140, and associated forms and correct fees, to the USCIS. Please note that there may be different addresses if you are filing only Form I-140 or if you are filing it concurrently with Form I-485.

    If the alien's employment will be in Arizona, California, Guam, Hawaii, or Nevada, mail the petition to:

    If the alien's employment will be in Alaska, Colorado, Idaho, Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Missouri, Montana, Nebraska, North Dakota, Ohio, Oregon, South Dakota, Utah, Washington, Wisconsin or Wyoming, mail the petition to:

    If the alien's employment will be in Alabama, Arkansas, Florida, Georgia, Kentucky, Louisiana, Mississippi, New Mexico, North Carolina, South Carolina, Oklahoma, Tennessee, or Texas, mail the petition to:
