Purpose of Form:
This package contains:
The Affidavit of Support Under
Section 213A of the Immigration and Nationality Act (Form I-864),
which must be completed by U.S. citizens and lawful permanent
residents who sponsor family members as immigrants to live in
the United States for immigrant visa and adjustment of status
applications filed on or after December 19, 1997.
The Affidavit of Support: Contract
Between Sponsor and Household Member (Form I-864A), to be used
in conjunction with Form I-864, when a sponsor is using the income
or assets of one or more household members in order to meet the
statutory income requirements of section 213A of the INA. By
signing this form, a household member obligates him or herself
to be joint and severally liable for the sponsored immigrant
with the sponsor until the obligation has terminated.
2002 Poverty Guidelines (Form
I-864P), to provide current minimum income requirements for use
in completing Form I-864.
Sponsor's Notice of Change of
Address (Form I-865), to be used, when necessary, by everyone
who has sponsored an immigrant pursuant to section 213A of the
INA, until the obligation of the sponsor to the sponsored immigrant
has been terminated. The sponsor is required to file Form I-865
within 30 days of each change of residence or mailing address.

Where to File:
Form I-864 and I-864A: Give the completed affidavit of support
with all required documentation to the sponsored immigrant for
submission to either a Consular Officer with Form OF-230, Application
for Immigrant Visa and Alien Registration, or an Immigration
Officer with Form
I-485, Application to Register Permanent Residence or Adjust
Status. This form must be signed before a notary public or
a Consular or Immigration Officer.
Form I-865: File this form with the Service Center with jurisdiction over the sponsor's new place
of residence.
