INTRODUCTION
This sample Animal Welfare Assurance is designed to aid institutions
in developing their Animal Welfare Assurances in accord with
the Public Health Service (PHS)
Policy on Humane Care and Use of Laboratory Animals. Foreign applicant
organizations and applicants proposing animal activity only at a collaborating
institution should not use this Assurance format, but should contact OLAW.
The sample includes all of the required elements of an Assurance.
Words that are italicized and in brackets [like this]
describe specific information to be provided by the institution
about its animal care and use program. All of the information
that is requested should be provided in a clear and concise manner.
Close adherence to the format of the sample will facilitate the
review process.
INSTRUCTIONS FOR ANIMAL WELFARE ASSURANCE
NOTES and SUGGESTED REFERENCES appear throughout
the sample. It is suggested that these notes and references be
deleted from an Assurance submitted to the Office of Laboratory Animal
Welfare (OLAW) since they are intended as guidance
to institutions in preparing their Assurances.

FOR DOWNLOADING
You may download the Sample Animal Welfare Assurance in MS Word
6.0 (.doc), WordPerfect 6.1 (.wpd) or Rich
Text Format (.rtf).

Determine which version of the sample assurance is best suited
for your Macintosh (Mac) or IBM Compatible computer (PC). |

We recommend the Word 6.0 version for visual appearance only
(the document was created in Word 6.0). All newer versions of
Word should be able to translate the Word 6.0 document. |

The WordPerfect 6.1 version includes the exact same content
and formatting as the Word version. PC users with earlier versions
of WordPerfect should see #5 below. Mac users can use the Word
version, or see #5 below. |

The general rule is that newer versions of Word and WordPerfect
can open documents created in earlier versions, however, older versions
cannot read documents created in newer versions. For example,
Word 2.0 can not open a document created in Word 6.0. |

Rich Text Format version (RTF) is a text file that will translate
tags (such as a bold and underline) when opened in a word processor.
The disadvantage of using RTF is that the formatting and tables
may be affected. If none of the versions below are compatible,
you should select the RTF version. |

Once you have decided on the appropriate version for your
word processor, select the link for it below by clicking it with
your mouse. Most current web browsers will either automatically
open the file in the appropriate word processor or provide a
"Save As" prompt. You may then save the file
on your hard drive or a disk. Once the file has been saved, you
will need to open the word processor application, and then open
the saved file.
