December 2002 Revised Long Term Care Resident Assessment Instrument User's Manual for the Minimum Data Set (MDS) Version 2.0


Description: The December 2002 Revised Long Term Care Resident Assessment Instrument (RAI) User's Manual for the Minimum Data Set (MDS) Version 2.0 replaced CMS's original Long Term Care RAI User's Manual Version 2.0 published in October 1995.

This version and subsequent updates of the RAI\MDS manual incorporate clarifications to existing coding and transmission policy, integrate previously published Questions and Answers (Q&As) into the appropriate sections and address requested clarifications and scenarios concerning problem areas.

Please note that InterRAI holds the copyright to Version 2.0 of the RAI for long term care outside of the US. Therefore, this revised Version 2.0 of the RAI/MDS manual should not be reproduced outside of the United States without permission of InterRAI. Within the US, Version 2.0 is in the public domain.

UPDATE FILES: The files listed here include only the pages from the RAI manual with the effected changes and can be placed directly into your manual. If you wish to download an entire updated chapter, see the Chapter sections below.

EFFECTIVE DATES: An update becomes effective on the day it is posted to the CMS website. Postings will occur on the fourth Monday of a month.

Final MPAF Form With OMB Form Numbers Added - No Changes to Instrument Items

The new MDS Medicare PPS Assessment Form (MPAF) (PDF 123K) will provide facilities with new options concerning the form used for Medicare PPS assessments. To allow reduction in assessment burden, facilities can choose to complete and submit the shorter MPAF form rather than a full MDS assessment for Medicare PPS assessments.

The OBRA assessment requirements supercede the use of the abbreviated form. When assessments are completed for both OBRA reasons and PPS reasons, then all OBRA-required items, all PPS-required items, and any state-specific items (Section S) must all be submitted, with all required items being stored in the state MDS database. When assessments are PPS-only (no OBRA reason present), only the MPAF items and any state-specific items (Section S) will be active and stored in the state MDS database. If a facility submits a full assessment for PPS reasons only, the extra MDS items (that are not on the MPAF form) will not be edited and will not be stored in the state database.

A document providing more specific detail regarding submission issues (PDF 105K)has been included. This is version 10 of this document (5/21/02). The only changes with version 10 are to correct the Medicare PPS reason for assessment values (AA8b) associated with Record-Type OM (bottom of page 1) and Record-Type AM (middle of page 2). For OM records, the appropriate values of AA8b are 1, 2, 3, 4, 5, 7, or 8. For AM records, the appropriate values of AA8b are 1, 5, 7, or 8.
