CERTIFICATE OF ATTENDANCE
"Certificates of Attendance" are available for those individuals who attended the National Leadership Summit on Eliminating Racial and Ethnic Disparities in Health, July 10 - 12, 2002, in Washington, D.C.
Please note that a certificate of attendance only verifies that you attended the Summit. Some individuals may be able to use the certificate of attendance as proof of training. The Summit did not offer CEU nor CME credits.
If you would like a certificate of attendance, please email your request to Ms. LeeAnn Robinson at:

- name as it appeared on your Summit registration;
- mailing address where the certificate should be sent (including organization, street address, city, state, zip code); and
- phone number with area code.
