The easiest way for beneficiaries to start or change direct deposit is to either contact their financial institution or call Social Security at 1-800-772-1213.

SSA does not have a form for direct deposit. The form used for direct deposit is a Treasury Department form, SF-1199a, available in Portable Document Format (PDF) at http://www.socialsecurity.gov/deposit/1199a.pdf.
