Third Party Internet Notice

If you are an Advocate, Attorney or Third Party Representative and you’re preparing or helping someone prepare an Internet Social Security Benefit Application and/or an Internet Disability Report, you must:

  • Not list your address as the mailing address for the applicant. List the mailing address of the person who is filing for benefits as the primary address in the application.
  • In the Summary/Remarks section at the end of the application, provide your:
    • name
    • agency name
    • complete mailing address, and
    • phone number.
  • Along with the applicant, complete and sign Form SSA-1696 (Appointment of Representative), if you’re the official representative of the person filing for benefits. Then, in the Summary/Remarks section of the Internet Social Security Benefit Application and/or Internet Disability Report, add:

    "A Form SSA-1696 (Appointment of Representative) is being forwarded to SSA on ____(date)_____."
