DLA 2004 Hall of Fame Nomination
Guidelines
1. Eligibility:
Induction into the Hall of Fame is open to all former DLA military and
civilian personnel regardless of rank/grade. Nominees must have been
separated from DLA for a minimum of one year and have no reasonable
prospects for future employment or assignment with DLA.
2. Admission Criteria:
Nominees are evaluated on their significant and enduring contributions
to DLA.
3. Nominations:
Any currently assigned, previously assigned, or retired DLA employee
may submit a nomination. Sponsorship by the Corporate Board and/or the
Field Activities is encouraged to facilitate the preparation and
justification of nominations, but is not required.
4. Sample Format:
Nomination packets must be complete when they are submitted. All
nominations must include the following:
a. Nominee’s name; service in DLA
including the nominee’s organization, position/title, and inclusive
dates of service; nominee’s current address (If nominee is deceased,
provide the data for surviving spouse or next of kin); and nominee’s
area code and phone number. General information on the person
preparing/submitting the nomination to include: name, current
organization/office, address, and area code and phone number.
b. A narrative description (no more
than 2 pages double-spaced) of the nominee's significant enduring
contributions to DLA.
c. A one-paragraph citation that
specifically highlights the nominee's accomplishments. This citation
will be read at the induction ceremony.
d. A portrait-style photograph of the
nominee (official photograph preferred), submitted electronically at
300 dpi.
5. Submissions:
All Hall of Fame nominations must be submitted electronically no later
than August 12, 2004. Send nomination packets to Kathryn Savelli at
