The mission of the EEOICP is to deliver benefits to eligible
employees and former employees of the Department of Energy, its contractors
and subcontractors or to certain survivors of such individuals, as provided
in the Energy Employees Occupational Illness Compensation Program Act. The
mission also includes delivering benefits to certain beneficiaries of
Section five of the Radiation Exposure Compensation Act.

APPLY
FOR COMPENSATION AND BENEFITS ON-LINE |
As of October 21, 2003, the Division of
Energy Employees Occupational Illness Compensation (DEEOIC) began
accepting the following claim forms electronically: Claim for
Benefits under Energy Employees Occupational Illness Compensation
Program Act (EE-1), Claim for Survivor Benefits under Energy
Employees Occupational Illness Compensation Program Act (EE-2),
Employment History under Energy Employees Occupational Illness
Compensation Program Act (EE-3) and Employment History Affidavit
for Claim under Energy Employees Occupational Illness Compensation
Program Act (EE-4). Claimants or affiants can complete forms
through the DEEOIC web site and then either mail them or – NOW! -
electronically submit them directly to the DEEOIC.
