Goal:
Our goal is to have all forms used Coast Guard-wide, including DOD, SF,
DOT, etc. Forms, automated by the year 2000. We're approximately 95% there. These forms
are accessible in the Jetform application on SWIII.
Forms that cannot be automated:
As much as we would like to become a paperless
Coast Guard, there are some forms that cannot be automated; at least not yet. They are
specialty forms i.e., posters, certificates (parchment paper), tags/tabs, books/pamphlets,
folders, ID cards, etc. These forms will either be stocked at Supply Center Baltimore or
in the "Office of Primary Responsibility" (OPR) at Headquarters.
How can you have your forms automated?:
Prepare and submit to G-CIM-2, a
CGHQ-3342, which is available in Jetform filler. Also, we need a filled-in copy of the
proposed form . The contractor will use this information to properly design or revise your
form, ensuring adequate fill-in space.
How to cancel a form:
Prepare and submit form CGHQ 3342 to G-CIM-2. Electronic
transmissions are acceptable
