For assistance with the Medicare Chronic Care Improvement Program National sample file data submit a written request to the Research Data Assistance Center (ResDAC) via their web based application found at https://resdac.oit.umn.edu/ . In your request indicate you are inquiring about the CCIP National Sample File.

The Research Data Assistance Center (ResDAC) is a CMS contractor that provides assistance to researchers interested in using Medicare and/or Medicaid data for their research. For the CCIP national file, ResDAC can assist with questions about Medicare claims data in general (i.e., inquiries regarding the origin and derivation of variables). Specific questions related to the technical aspect of the CD, such as how to link the various tables, should be posed to the CCIP email address ccip@cms.hhs.gov.
