In order to become an approved FHA Title I or Title II lender, an application package must be submitted to the Office of Lender Approval and Recertification in HUD Headquarters for review. An application package can be obtained by calling (800) 767-7468 or faxing your request to (202) 708-2313. Ask for the Title I Application Package - Stock No. 4112 or Title II Application Package - Stock No. 4113.

If you have any question regarding your application the Lender Approval and Recertification Division can be reached by telephone at (202) 708-3976, by fax at (202) 755-3003 or e-mail at HSG-Lender_approval@hud.gov. Due to increased volume, we suggest you email.
