System. The

provide important information on data sources, statistical methods used,

Mortality
Technical Notes
These tables present descriptive
tabulations of information reported on the death certificates. Death
certificates are completed by funeral directors, attending physicians,
medical examiners, and coroners. Original records are filed in the state
registration offices. Statistical information is compiled into a national
data base through the Vital Statistics Cooperative Program of NCHS. For a
full description of the data and random variation, see the NCHS mortality
website.

Natality
Technical Notes
These tables present descriptive
tabulations of information reported on the birth certificates. Standard
forms for the collection of the data and model procedures for the uniform
registration of the events are developed and recommended for State use
through cooperative activities of the States and NCHS. Statistical
information is compiled into a national data base through the Vital
Statistics Cooperative Program of NCHS. For a full description of the

Risk Factor Surveillance System
The Behavioral Risk Factor Surveillance System (BRFSS) tracks health risks
through continuous telephone surveys by each state. Major risks
associated with premature morbidity and mortality among adults are
