Survey and Certification

CMS maintains oversight of the survey and certification of nursing homes and continuing care providers (including hospitals, nursing homes, home health agencies (HHAs), end-stage renal disease (ESRD) facilities, hospices, and other facilities serving Medicare and Medicaid beneficiaries), and makes available to beneficiaries, providers/suppliers, researchers and State surveyors information about these activities.

  • General Information--Includes Updates, Overview of S&C, Accreditation/Deeming, Quality of Care, Public Statements and Contact information.

  • Background Checks Program

  • Beneficiaries--Includes Medicare/Medicaid Overviews, Quality Care Reports, Fraud and Abuse (Medicare), Abuse and Neglect (Medicaid), and Contact Information.

  • State Agencies & Regional Offices--Includes State Survey Agency Directory, and State Performance Standards.

  • Surveyors--Includes Learning Resources, Forms, and other Surveyor specific information.

  • Researchers--Includes Survey & Certification research data and materials.

  • Providers/Suppliers--Contains general provider resources and specific information for Nursing Homes, Hospices, Intermediate Care Facilities for Persons with Mental Retardation (ICF/MR), Home Health Agencies, Organ Procurement Organizations (OPOs), End-Stage Renal Disease Facilities (ESRDs), Psychiatric Hospitals, Rehabilitation Services, Acute Care Providers/Suppliers, Critical Access Hospitals.
