Nominations Being Accepted for the Robert Wood Johnson Community Health Leadership Program (CHLP) Awards

Letter of Intent Deadline: September 22, 2004

Do you know someone who finds creative ways, despite complex odds, to bring health services to their communities?

If so, consider nominating him/her for the Robert Wood Johnson Community Health Leadership Program’s (CHLP) annual leadership award. Ten honorees will be selected from across the United States to receive $120,000 each, of which $105,000 is earmarked for program support and leveraging other resources. The remaining $15,000 is slated for use as a personal stipend.

Nomination Criteria
Nominees must be U.S. citizens or residents living and working in the United States, District of Columbia or Puerto Rico. Candidates must work and/or volunteer three-quarters or full-time in community-based organizations, and have gone above and beyond the call of duty for their local areas.

Please Note: Only nominations of "unsung heroes," who have yet to receive a major honor or award, will be accepted.

Nomination Process
The nomination process begins with the submission of the Letter of Intent (LOI) form by September 22, 2003. LOI forms may be submitted online. Otherwise, nominators may download the printable HTML, Microsoft Word or PDF LOI form from the site and submit it to the following address:

Approximately four weeks after receipt, CHLP will notify nominators whether or not their candidate has been deemed eligible for the award, and will receive a nomination packet containing the instructions and forms required to complete the award application process.

Nominations are due November 10, 2003.

Nominators whose candidates were not selected will receive a letter of explanation.
