Once you have opened the order form in Adobe Acrobat Reader you can choose to either:
1. Print the order form and then fill in the blanks either by writing clearly (in English) or using a typewriter.
OR
2. Save the blank order form on your computer and fill out the fields by clicking above the lines which require your information. To advance to the next field while typing in the order form just press the "tab" key. After you have filled in all fields, print, and sign the order form.
PLEASE NOTE: The order form cannot be filled out partially and then saved with the free Acrobat Reader. You can still print the form immediately after you have filled it in with the free Acrobat Reader. To save a completed order form you will need a full version of Adobe Acrobat.
Payment Information
Individual orders, examination requests, and relevant
international orders (see Foreign Orders for territories) must
be prepaid in U.S. funds payable on a U.S. bank. Please make checks
or money orders payable to: Endowment of the U.S. Institute
of Peace. Credit card orders Mastercard
and VISA only) are also acceptable--$10.00
minimum. Please do not send cash. Information about trade and
wholesaler discounts will be furnished on request: the letter
“s” following some prices indicates a short discount.
Prices listed apply to orders filled from our Virginia warehouse and
are suggested U.S. retail. Prices and availability are subject to
change without notice.
To receive a copy of a paperback book
being considered for course adoption (U.S. orders only):
- Download the form.
- Complete all course information for each title requested
(see form).
- Furnish complete institution address/shipping information.
- Enclose shipping fee of $4.00 per title. A maximum of 3
titles may be requested.
Please enclose a check or money order. (Credit cards
are not accepted for examination requests.)
Examination requests and individual orders may be
combined by filling out both the book order form and the examination
request form. Be sure to fill in the shipping information areas
on both forms. Please note that all exam requests, individual
orders, and overseas orders must be prepaid.
