MT-1: Application Forms for Conditional and Final Letters of Map Amendment and Letters of Map Revision Based on Fill
The application forms included in these files were designed to assist requesters (community officials, individual property owners, and others) in gathering the information FEMA needs to determine whether property (parcel[s] of land or structure[s]) is likely to be flooded during the flood event that has a 1% chance of being equaled or exceeded in any given year (base flood).
The forms in this package shall be used to request Letters of Map Amendment (LOMAs), Conditional Letters of Map Amendment (CLOMAs), Letters of Map Revision Based on Fill (LOMR-Fs), and Conditional Letters of Map Revision Based on Fill (CLOMR-Fs)
The files are available for download in WinZip and Adobe PDF format. The instructions are also available in text (.txt) format.
