Grants to States to Foster Infrastructure to Assist Individuals to Live in the Community

Date Certain Grant 1998

1998 was the first year for the grant initiative, and awards of approximately $187,500 each were made to Texas, Michigan, Colorado, and Rhode Island. The focus of the grants programs in these States were to either transition individuals from the Nursing Home to the community (if that is their choice), or to divert unnecessary Nursing Home placement utilizing interventions during discharge planning. Grant moneys were used to: hire staff to enhance infrastructure, develop and test discharge forms, assist individuals by providing needed costs such as resources for moving into an apartment (down payments), perform outreach efforts in the community, identify and eliminate barriers for community living, etceteras.

Nursing Home Transition Grant 1999

Continuing on the program that began in 1998 (see above), the grant solicitation requested applications for programs that focused on transitioning current Nursing Home residents back into the community (rather than diversion). Awards of approximately $500,000 each were made to New Jersey, Vermont, New Hampshire, and Wisconsin.

Nursing Home Transition Grant 2000

Similarly to the 1999 focus, the 2000 solicitation again included applications for programs that focused on transitioning current Nursing Home residents back into the community (rather than diversion). Awards of approximately $500,000 each were made to Pennsylvania, Arkansas, Nebraska, and Florida.
