Confidential Financial Disclosure Annual Campaign
Filing Deadline: October 31

For your convenience, you have the option of using one of several versions of the form OGE 450, Confidential Financial Disclosure Report, through links with the USDA Office of Ethics, an ADOBE PDF version, or completing and saving your report online through NFC. This filing cycle, the short form, OGE 450A, Confidential Certificate of No New Interests, will be accepted from employees who have no changes from filing last year's 450 with REE. Please complete the form, sign and date the form in ink, and submit to your Ethics Advisor no later than October 31.

Note: ARS-HQ, CSREES, ERS and NASS employees should submit their confidential financial disclosure reports directly to the REE Ethics Office, 5601 Sunnyside Avenue, Room 3-1170A, Beltsville, MD 20705-5102.

All financial disclosure filers must also receive prior approval before engaging in any non-Federal employment or activity. Please complete the form REE-101, Application for Approval to Engage in Non-Federal Employment or Activity, found at: www.afm.ars.usda.gov/hrd/ethics/outside.htm

OGE 450 -- Confidential Financial Disclosure Report -- to be used by anyone in a covered position, either designated by their position description or by a collateral duty (ADO, ADODR, CO, COR or COTR), or who is responsible for making financial decisions which may impact contractors or non-Federal interests that are regulated by the Department, is a Purchase Card Holder with monthly purchasing authority exceeding $25,000, or is a Special Government Employee (Collaborator, Consultant, Expert or Panel Manager).

OGE 450A - Confidential Certificate of No New Interests -- to be used only when there have been no changes from filing your last 450 with REE. This form may not be used by SGEs (Collaborators, Consultants, Experts, or Panel Managers) or by first time filers with REE.

NOTE: This form will not be available during Financial Disclosure campaigns in an election year. Only OGE Form 450 will be accepted from October 1, 2004 through September 30, 2005.

Online Reporting -- to complete your financial disclosure report (OGE-450) through your Personal Page with the USDA National Finance Center. If you choose to complete your report online, please print the report, sign it, and mail it to your ethics advisor. This option allows you to go in at a future date to amend your report without having to reconstruct your report from scratch.
