Linfoma difuso de células pequeñas no hendidas/linfoma de Burkitt's
El tratamiento de estos linfomas generalmente se realiza a través de regímenes
agresivos de múltiples fármacos similares a los usados para los linfomas en
estadio avanzado (difusos de células grandes).[1-3] Se ha descrito un ensayo clínico intensivo utilizando quimioterapia en combinación agresiva
estructurada de acuerdo a la usada en el linfoma de Burkitt's infantil y ha
tenido mucho éxito en pacientes adultos.[4-8] Los factores de pronóstico
adverso incluyen la enfermedad abdominal voluminosa y deshidrogenasa láctica
sérica elevada. En algunas instituciones, el tratamiento incluye el uso del
trasplante de médula ósea de consolidación.[9,10]
Los pacientes que tienen linfoma difuso de células pequeñas no hendidas/linfoma de Burkitt's tienen entre 20% y 30% de riesgo de por vida de tener
complicaciones del sistema nervioso central (SNC). Se recomienda la profilaxis
del SNC (por lo general con 4-6 inyecciones intratecales de metotrexato) para
todos los pacientes. En una serie de 41 pacientes tratados con quimioterapia
sistémica e intratecal, 44% de los que se presentaron con enfermedad en el SNC
y 13% de los que tuvieron recurrencias con compromiso del SNC se convirtieron
en sobrevivientes a largo plazo sin enfermedad.[11] Los patrones de recidiva
fueron similares independientemente de que el paciente fuera irradiado o no,
pero hubo un aumento en déficit neurológico en aquellos que fueron irradiados.
