    N-426, Request for Certification of Military or Naval Service

    Instructions:

    Purpose of Form: This form is used by an applicant for naturalization to request that the Department of Defense verify the applicant's military or naval service.
    Number of Pages: 3 sets of 2 pages, to allow the form to be filled out in triplicate. 1 page of instructions. Total printed pages: 7
    Edition Date: 5/28/04. Other editions are acceptable only through 9/30/04.
    Where to File: Return the form to the office which requested the certification.
    Filing Fee: None

     

    Special Instructions:

    Pages 1, 3 and 5 of the form are identical and are to be filled out by the applicant.
    Pages 2, 4 and 6 are to be filled out by a certifying officer of the Department of Defense.
