Lender Forms and Reports

Lenders are required to submit quarterly student loan portfolio data to receive payments and to report origination/lender fees due to the Department of Education (ED). ED will pay the interest on eligible FFEL loans that are in-school, grace or authorized deferment. ED will also pay special allowance on the life of all eligible FFEL loans. These payments are based on the transmission of  valid financial data via the Lender Reporting System (LaRS).

The forms listed below are available for lenders to view and/or download. 

Lenders' Interest and Special Allowance Request and Report--ED Form 799 (for historical purposes only)

Prior to implementation of the LaRS, lenders and servicers used the hardcopy ED Form 799 to submit quarterly loan information reporting origination and lender fees collected, interest and subsidized loans that qualify for grace and authorized deferment and special allowance payments for the life of the loan.

 

Federally-Insured Student Loan (FISL) Claims Request--ED Form 1207 [XLS - 1 page] (submitted to ED as needed)

Federally-Insured Student Loans (FISL) issued to students were guaranteed by the Department of Education until June 1984. FISLs were similar to today's Stafford loans, but the Department provided the loan guarantee to lenders instead of a GA. When students default on their FISL loans, the lenders file claims directly with the Department requesting payment for the outstanding balance due.
