Notice to CDC Grantees
SAS License Request Instructions

Who can request SAS products?

According to the SAS license agreement, any CDC grant recipient may use SAS products for "Program Activities". Program Activities consist of the use of SAS Institute products for the development, deployment and operation of computer applications or information systems for the identification, collection, management, reporting, tracking, and analysis of the disease, health event, health condition, and health determinant data required for national systems of surveillance or research in order to fulfill the mission of the CDC/ATSDR Program Activities as specified in the applicable grant or cooperative agreement between CDC/ATSDR and the grantee.

For the purpose of this license agreement, a Grantee is defined as the local or state agency (e.g. STD program) that receives funding from official grants or cooperative agreements from CDC for the purpose of conducting disease surveillance. Sub-recipients (e.g. city or county STD programs within a state) may not individually request SAS products through this mechanism. However, the designated authority within the agency receiving the grant may make this request, if appropriate, so that the requested SAS products are distributed to the sub-recipients.

What SAS products are available?

A detailed list of the available SAS products is provided in SAS CD Sets Available to CDC Grantees. Because of the extensive nature of this list, STD project areas are strongly encouraged to request only those SAS products they need. Additional requests can be made later if needs change.

What are the SAS license requirements?

CDC grant recipients may request SAS products via this mechanism only after acknowledging and accepting the terms of this license agreement. Please refer to CDC-SAS License Provisions for a complete list of the terms of this agreement.

In addition, quarterly reports that document the SAS product usage are required. For details of this reporting requirement refer to SAS Institute Products Requirements and Reporting Procedures .

How to request SAS products?

The following steps describe the process that must be followed for requesting SAS products under this CDC license agreement. Please note that the digital certificate request and SAS web page access are limited to users of Microsoft Internet Explorer, version 5.0 or higher. Netscape Navigator is not supported for this application.

1. The STD program manager/administrator sends an email (to Sdanos@cdc.gov) that indicates a desire to request SAS products from CDC under this new license agreement. The email must include the CDC grant number and location requesting the software (e.g. State Health Department, Local Health Dept., etc.). Do not list the full mailing address here, just the name of the agency and department that wish to receive the SAS products. When the request is made from anyone else (e.g. STD Epidemiologist or surveillance coordinator), please include the name and phone number of the STD program manager/administrator.

2. If eligible, you will receive an email that provides the instructions to be followed to request a digital certificate from CDC. This is part of the CDC Secure Data Network (SDN) and is used to authenticate the person and computer within the CDC network.

3. Once the digital certificate has been received and installed, access the SDN web site (https://sdn.cdc.gov/) using the computer that now has the digital certificate. Choose Request Software under the CDC SAS LICENSING ACTIVITIES menu.

4. Complete the CDC SAS LICENSING SYSTEM web page. Please note that multiple SAS products may be requested. Be sure to "ADD" each needed product before submitting the request. Complete one web page request form for each location where the SAS products will be shipped.

5. You will be notified by email when your request has been approved and when the products have been shipped to the address specified in the web request form.

What about SAS Training and Product Support?

The Division of STD Prevention is not staffed with the resources needed in order to provide training and support for these products. These services can be obtained from numerous other sources under separate contract, including the SAS Institute, but STD grantees should not plan on CDC assistance in the use of these products. In addition, this license agreement states that the SAS Institute will not provide training or support of SAS products distributed to CDC grantees under this agreement.
