The following are some tips that will assist you in determining your Medicare eligibility and enrollment status:

My Medicare Enrollment

This section of the site is designed to provide you with information about your Medicare eligibility and enrollment. You will be asked to answer a series of questions. If you are not a U.S. citizen or a lawfully admitted alien who has lived in the U.S. continuously for a five-year period, please contact the Social Security Administration at 1-800-772-1213 for your Medicare enrollment and eligibility.

Eligibility & Enrollment Date Calculator

This calculator will provide you with the dates that you will be eligible to enroll in Medicare. If you are over 65 and/or already enrolled in Medicare, please contact the Social Security Administration at 1-800-772-1213 for further information on your Medicare eligibility.

Medicare Part B Surcharge Calculator

This calculator will provide you with an estimate of the surcharge on your Part B premium based on the 2004 premium amounts. If you are under age 65, disabled or if you had Part B before and dropped it, please contact the Social Security Administration at 1-800-772-1213 for specific information about your Part B premium amounts including any surcharges. If you didn’t take Medicare Part B when you were first eligible because you or your spouse were working and had group health plan coverage through your or your spouse’s employer or union, please contact the Social Security Administration at 1-800-772-1213 for specific information about your Part B premium amounts including any surcharges.
