Federal Employees' Group Life Insurance Program: Revisions of FE-6, Claim for Death Benefits, and FE-6 DEP, Statement of Claim-Option C Family Life Insurance

Why did the Office of Federal Employees' Group Life Insurance (OFEGLI) revise the forms FE-6 and FE-6 DEP?

OFEGLI revised the forms to:

  • update Internal Revenue Service-required backup withholding language and other certification information.
  • lower the dollar amount for depositing payments into a money market account to $5,000 to make this benefit available to more claimants.

What are the revision dates?

The revised forms are dated April 2004.

How do we obtain the forms?

Since agencies distribute these forms infrequently, we recommend that you download copies of these forms on an "as needed" basis. You can download the revised forms from the FEGLI web site (http://www.opm.gov/insure/life) in a PDF-fillable version.

Can we use previous editions?

No. You should disregard all previous editions since new information has been added to the forms.
