The 5th Immunization Registry Conference
(IRC) will bring together over 450 local, state, federal, and private
sector immunization registry partners, to promote knowledge and
information about the development and use of immunization registries.
The goal of the conference is to provide participants with programmatic,
technical, and scientific information that will improve the use
of immunization registries. The conference will also provide a forum
to build support for immunization registries, enhance collaboration,
promote multiple and innovative uses of registry data, explore alternative
funding strategies, and demonstrate immunization registry successes.
Participants will identify benefits and roles for all participants
in registry development and use, including health care providers,
public health officials, and families.
During three days of plenary sessions
and workshops, many topics will be discussed, including:

Official conference activities are
tentatively scheduled to begin at 8:30am on Monday, October 18 with
the Opening Plenary Session in the Main Ballroom at the Crowne Plaza
Ravinia Hotel. The Conference is scheduled to end at 12:30pm on
Wednesday, October 20.
PLEASE NOTE: the
DRAFT Conference Agenda is subject to change at

Guidelines
for Preparing Your Presentation |
The Conference Planning Team is pleased
to provide information and guidelines that will be useful to you
as you prepare for your Poster or Workshop presentation at the 5th
Immunization Registry Conference.
Top
Continuing education credits will
be offered for various professions based on 13 hours of instruction.
All continuing education
credits and Certificates of Attendance for the 5th Immunization
Registry Conference will be issued through the CDC/ATSDR
Training and Continuing Education Online system.
To receive CE credit or obtain a
Certificate of Attendance, participants must go to the online system
to register and complete the evaluations after attending sessions.
Users will be able to print their certificates from the online
system. Computers
will be available during the conference in the Ashford Room for
participants who wish to complete their evaluations on-site.
Participants who do not complete their evaluations on-site
will have until November 22, 2004 to complete the
online evaluations from any Internet-connected computer.

Immunization Registry Conference has closed. If you have not already

INFORMATION about the conference SLEEPING ROOM
RATE: Please note
that the specially-negotiated conference room block rate at the
headquarters hotel is tied to the Federal Government FY 2005 per
diem room rate, which is set by the General Services Administration.
If you stay at the conference hotel or overflow hotel in a room
booked as part of the conference room block, you will be
charged the prevailing per diem rate for the Atlanta area,
plus applicable taxes, regardless of what the per diem rate was
at the time you booked your reservation. This rate is
subject to change without notice; it has changed twice
already this year. As of October 6, the FY 2005 Federal Government
per diem rate is $113 plus applicable taxes.
As of October 6, a limited number
of rooms are available at the headquarters hotel. The headquarters
hotel for the 2004 Immunization Registry Conference is the Crowne
Plaza Ravinia Hotel in Atlanta, Georgia
(located at 4355 Ashford Dunwoody Road, Atlanta, Georgia 30346-1521).
To make a reservation at the conference rate, please call the hotel
at 770-395-7700 or 1-800-554-0055.
Identify yourself as a member of the "CDC/5th Immunization
Registry Conference" group. You
can also reserve
your hotel room on the Internet. To reserve your room
at the specially-negotiated conference room rate, be sure to enter
Group Booking Code IMM in the reservations form
on the left-hand side of the hotel reservations page. Check-in
time at the hotel is 3:00pm; check-out time is 12:00pm. Cancellations
will be accepted without penalty until 6:00pm local time on the
scheduled day of arrival.
In addition,
as of October 6, a limited number of
overflow rooms are available
at the Hilton Garden Inn Atlanta Perimeter Center (located
less than 5 minutes from the conference hotel; with free parking
and a free shuttle to the conference hotel). To make a reservation
at the overflow hotel at the specially-negotiated conference rate,
please contact the hotel at 404-459-0500 and ask
for the "Immunization Registry" rate.

Event Registration |
The Conference Planning Team is pleased
to offer a Rhythm Aerobics Fitness Event for conference
attendees. Come and enjoy a
high-energy cardio workout with a touch of funk. You will love the
instructor's energy and the workout's upbeat music. You are guaranteed
a great time and an awesome workout. This event is geared for all
fitness levels.
The Fitness Event will take place
on Tuesday morning, October 19, 2004, at the conference hotel. The
first 75 registered participants in the conference fitness event
will receive conference t-shirts. Pre-registration is closed; you
can still register on-site at the conference at the Information
Desk.
