Provider Reimbursement Manual
Part I

The files below recreate a relatively complete version of the Provider Reimbursement Manual Part I. A few pages here and there that were never produced in Microsoft Word 97 format (e.g., certain charts, graphs, diagrams) could not be included. Otherwise, every effort has been made to ensure the accuracy of these documents in electronic form. However, the paper version is considered the official version and takes precedence if any discrepancies occur.

NOTE: The files listed above are ZIPPED files. ZIPPED files must be downloaded to your PC before they can be unzipped. This format saves disk space and time needed to download. (This process will only work if you have WinZip on your PC. You can purchase the WinZip software at WinZip.com, or go to the Web and get a free trial vision that will only last 29 days.)

To download, extract and view the file(s), please follow the instructions below:

  1. Create a directory on your PC for the downloaded file.
  2. Click on the hypertext link for the zipped file, to start the download process.
  3. Select "save as file" (DO not select "open") when requested by your browser software and indicate the location where you wish to save the file, (i.e., disc, C:, etc.).
  4. Once the file is saved to your specified location, you must first extract the files it contains before you can view them. To do this, go to your "file manager" ("explore" in Windows 95), and locate the file. Double click on the file name to start the self-extraction process. When completed, there will be one or more additional files, located in the same directory as the original downloaded file.
  5. The files may be viewed using your word processing software, (e.g., Microsoft Word 97, WordPerfect version 8, etc.). Files ending with the "xls" extension may be viewed using Microsoft Excel version 5 or higher spreadsheet software.
