Internet Data Entry System for  Data Collection Form for Reporting on Audits of States, Local Governments, and Non-Profit Organizations

The Internet Data Entry System (IDES) is an automated data entry system for the Form SF-SAC. The user will enter Form SF-SAC information (including manually typing up to 80 program lines or uploading an unlimited number of program lines), then check it for errors, and print a final form for submission. The timesaving error check function will eliminate many Form SF-SAC rejections before they occur.

To start entering data, click on Enter a New Form button. The IDES will assign a unique identification number that you will need to retrieve the saved information. Use the button Update a Form to retrieve previous session information.

Once the form is complete and no errors exist, print the form and have the auditor and auditee sign and date the form. Mail the signed form with the proper number of reporting packages to the following address:
