EPO presents an electronic way for past and present EIS officers to provide their current information for the annual EIS Directory. This method was created to make updating the information in the Directory easier, both for the staff and for the officers. The forms may be accessed from any computer with a connection to the Internet. Because the information is e-mailed directly to the EIS Directory inbox and not stored on your computer, the information is kept secure.

BEFORE YOU FILL OUT A FORM
Our most current information available is in the 2002-2003 Directory. If it has changed since then and you don't have the Directory to check, please send an e-mail to eisdirectory@cdc.gov requesting your information.
