raise awareness of immunization. It also
explains a four-step process to plan and
implement programs that can help raise and
sustain immunization rates in your community.
The Community Guide also includes sample
documents, such as a letter, proclamation,
press release, and sample program plan that
can be tailored to fit your individual needs.
Throughout the Community Guide, sidebars
contain tips on conducting activities effectively.

The Implementation
Handbook consists of a variety of individual
reproducible materials. Each page is perforated
for removal. Your organization may wish to
duplicate these pages for distribution to
parents, providers, and other interested
