Electronic Forms LM-2, LM-3, LM-4


You can complete the Labor Organization Annual Report, Form LM-2, LM-3, or LM-4, quickly and easily using your computer and the electronic forms software developed by the U.S. Department of Labor’s Office of Labor-Management Standards (OLMS).

Union officers who wish to submit their LM Report to OLMS over the Internet must use a digital signature to electronically sign the report. For information on the procedures and cost of obtaining a digital signature, go here.

Please note:

For reports for fiscal years beginning before July 1, 2004, the simplified annual report Form LM-3 may only be used by unions with total annual receipts of less than $200,000, if not in trusteeship. For fiscal years beginning July 1, 2004 or later, Form LM-3 may be used by unions with less than $250,000 in total annual receipts, if not in trusteeship.

This Form LM-2 may only be used for fiscal years beginning before July 1, 2004. For fiscal years beginning on July 1, 2004 or later, LM-2 filers must use the electronic Form LM-2 located here.

Why should I use the electronic forms?

  • It’s easy to enter information on the form.
  • You can download some information from reports previously filed by your union directly onto the form.
  • If you are filing Form LM-2 or LM-3, you can import electronic officer and employee information directly onto the forms.
  • The electronic forms software performs calculations for you, so you won’t make mathematical errors.
  • The system checks for errors and discrepancies and allows you to correct or explain them, reducing the likelihood of having to file an amended report.

How can I obtain electronic forms software?

You can download the latest version of the OLMS Electronic Forms and any plug-ins or updates to insure you have the most complete Electronic Forms Software from the OLMS Download Page.

If you are unable to download the Electronic Forms Package you may obtain the software on a CD by calling 1-202-693-0123 or the nearest OLMS field office.

Please note that for fiscal years beginning July 1, 2004 or later, the Revised Form LM-2 must be filed by every labor organization with $250,000 or more in total annual receipts.

Can I submit my completed form electronically?

Union officers can now sign their LM Report electronically using a digital signature and submit it to OLMS over the Internet. Go to the OLMS Digital Signature Page for detailed information on obtaining and using digital signatures.

Please note that Revised Form LM-2 filers (labor organizations with fiscal years beginning July 1, 2004 or later with $250,000 or more in total annual receipts) are required to file electronically.

How can I get assistance in using the electronic forms software?

The OLMS Help Desk staff provides assistance with any questions or difficulties you may have using the software. The Help Desk is staffed during normal business hours and can be reached by calling (toll-free): 1-866-319-2675.

Please note that the OLMS Help Desk is equipped to provide assistance to filers of the Electronic Form LM-2 only for fiscal years beginning before July 1, 2004, but to all LM-3 and LM-4 filers.
