These materials were developed by FDA's Office of Women's Health in collaboration with the National Association of Chain Drug Stores and the American Diabetes Association.

ALERT CARD

Diabetes is serious.
To avoid heart and kidney problems, blindness, stroke, the loss of a foot or leg:

  1. Watch what you eat and get exercise.
  2. Use medicines wisely.
  3. Check your blood sugar.

Print the Alert Card with this message and a place to print your name, insurance number, doctor's name, telephone, and pharmacy in case of emergencies. The card is also available in Spanish.
