Forms
Most forms that are required for wage reporting are IRS forms. However, SSA does have a few forms that are useful to employers:
SS-5 - Application for a Social Security Card
SSA-7004 - Request for Earnings and Benefit Estimate Statement
Form SSA-131, Employer Report of Special Wage Payments
Form 6559, Transmitter Report and Summary of Magnetic Media (PDF)
Form 6559-A Continuation Sheet for Form 6559 (PDF)
NOTE: Forms SSA131 and Form 6559/6559-A require that you have Adobe's Acrobat Reader.
IRS Forms
The IRS has several forms that are necessary for wage reporting. The IRS makes these forms available in four different formats. The easiest format to access is Portable Document Format accessible with the Adobe Acrobat Reader. If you do not have this software, see our information on Adobe's Acrobat Reader.
After you have downloaded and installed this software, you can find all of the following forms at the IRS's Forms Page.
