Physical Activity Program Directory
Comments and Inquiries
In order to make this directory as accurate and useful as possible, we
need your input! Please send us feedback about the general
usefulness of the search functions and content of the directory. If
you contact us about changes in programs that are already in the directory
or about additional programs, please include the following information:
- program name
- partners
- beginning and ending dates
- scope (geographic area served)
- target population (including age, race/ethnicity, specific
populations, etc.)
- setting
- program components (e.g., educational classes, media advocacy,
public awareness, social support, etc.)
Also, please provide one sentence about the general purpose of the
program and briefly describe any evaluation or products (e.g., curricula,
videotapes, etc.) associated with the program.
