SUBJECT: ENROLLMENT OPPORTUNITY FOR CERTAIN HEALTH ALLIANCE BENEFIT PLAN (CODE FX) ENROLLEES The Health Alliance Benefit Plan (Code FX) will lose most of its primary providers in the Quincy (western Illinois) and Peoria (central Illinois) service areas. OPM, therefore, is authorizing an opportunity for affected Health Alliance Benefit Plan enrollees to change their FEHB enrollments.
QUINCY SERVICE AREA As of August 31, 2000, coverage in the Quincy service area will no longer be offered through the Health Alliance Benefit Plan for the following counties: Adams, Brown, Pike and Schyler. Note that enrollees residing in Hancock County (also located in the Quincy service area) will not be affected. The opportunity to change FEHB enrollment for those covered by Quincy service area will begin immediately and continue through the last day of the pay period that includes August 26, 2000. The effective date of all enrollment changes will be the first day of the first pay period beginning on or after August 27, 2000.
PEORIA SERVICE AREA As of December 31, 2000, FEHB coverage in the Peoria service area will no longer be offered through the Health Alliance Benefit Plan for the following counties: Fulton, Henry, Knox, Marshall, Peoria, Putnam, Stark, Tazewell, Warren and Woodford. The opportunity to change FEHB enrollment for those covered by the Peoria service area will begin immediately and continue through the last day of the pay period that includes September 23, 2000. The effective date of all enrollment changes will be the first day of the first pay period beginning on or after September 24, 2000.
