State-Based Diabetes Prevention & Control Programs

North Dakota

Image of the state of North Dakota The North Dakota Diabetes Prevention and Control Program (DPCP) began receiving funding from the Centers for Disease Control and Prevention in 1987. Activities supported by the North Dakota DPCP include the following:

Ongoing surveillance. The North Dakota DPCP analyzes data from a variety of sources including the Behavioral Risk Factor Surveillance System, Vital Records, insurance vendors, and clinics. Statewide public and private partners such as North Dakota Health Care Review, Inc., the American Diabetes Association - North Dakota Affiliate, Blue Cross Blue Shield of North Dakota, and many health systems cooperate in diabetes surveillance efforts.

North Dakota Diabetes Care Provider Report. Through a unique public-private partnership, the DPCP and Blue Cross Blue Shield monitor diabetes care through a claims-based assessment system. Diabetes care providers receive quarterly reports detailing progress in quality care indicators for their patients who have diabetes.

Diabetes Collaborative. The North Dakota Diabetes Partnership is working with Dakota Medical Foundation in a statewide diabetes collaborative based on the Bureau of Primary Health Care model. The collaborative is designed to promote quality diabetes care in North Dakota hospitals and clinics.

Diabetes Today. A core group received training in the Diabetes Today method of community-based diabetes education. This train-the-trainer program emphasizes diabetes control and prevention complications. Community and fraternal organizations are used to spread diabetes care and prevention messages.

Public Awareness. The North Dakota DPCP partners with the American Diabetes Association - North Dakota Affiliate and the North Dakota Diabetes Partnership to disseminate diabetes prevention and care messages to the public. Awareness campaigns target Diabetes Alert Day in March and November's National Diabetes Month.
