Evaluation
The REACH 2010 Demonstration Program evaluation is critical in
determining the program's effectiveness in reducing health disparities. CDC, with input
from grantees and partners, has developed an evaluation logic model
to guide the collection of national data. The model evaluates programs on
their effectiveness in the following areas:
- Building Community Capacity
- Developing Targeted Actions
- Improving Health Systems and Agents of Change
- Increasing Protective Behaviors and Reducing Disparity-Related
Illness and Death
- Health Disparity Reduction
The University of South Carolina, one of CDC's Prevention Research
Centers, was selected to manage a Special Interest Project (SIP) entitled
CENTERED (Community Evaluation Networks Targeting Entrenched Racial and
Ethnic Disparities). The CENTERED group, comprised of members from South
Carolina's Department of Health and Environmental Control, and members of
the highly diverse Blue Ribbon Panel that includes academics,
representatives of community-based organizations, and experts from public
and private sector organizations, designed the evaluation process for
REACH 2010 and other projects aimed at eliminating health disparities. The
CENTERED group has agreed to develop evaluation methodologies and national
evaluation support networks to assist community-based public health
approaches to health disparities elimination. The CENTERED Web site,
