Clinical Laboratory Improvement Amendments (CLIA)

This CLIA laboratory demographics file is populated with data queried from the Centers for Medicare & Medicaid Services CLIA OSCAR database at intervals of approximately 6 months. The file provided is therefore a static view of the data and may not reflect the most current information available. Laboratories that would like to update their information in the HCFA database should contact their State Survey Agency.
 
WARNING: This file contains ~ 170,000 records in a compressed comma delimited text file. The compressed file size is ~5.5 Mb and may require a significant amount of time to download (depending on network traffic and connection speed). The uncompressed file will require ~ 19 Mb of disk space. Once downloaded, simply double click on the file name to uncompress the data. The resulting comma delimited file may be imported into a variety of database and spreadsheet formats. Familiarity with importing comma delimited files is recommended.
