Submission Procedure

Note to J-1 Waiver Applicants

On March 15, 2000, this agency was advised that recommendations for waiver of the two-
year foreign residence requirement that we forward to the Department of State must include a
case number. Applicant organizations can obtain that case number by following the Department
of State instructions below.* This new procedure was effective March 31, 2000.

Department of State Procedures for Waiver Applications

To apply for a recommendation for a waiver of the two-year home residence requirement,
applicants must complete a Data Sheet (available via the Internet at
http://travel.state.gov) and
send the completed Data Sheet, two self-addressed, stamped, legal-size envelopes (S.A.S.E.),
and a check or money order for $230 per application, payable to the U.S. Department of State, to:

Any cases now pending before the Department of Health and Human Services will be
required to have this case number before an approval can be forwarded to the Department of State.

Applicants to HHS have the option of obtaining the case number before submitting the
application to HHS or waiting to obtain the case number after HHS makes its decision. We
understand it may take up to eight weeks to obtain a case number.
