Physician Profile, Appendix 5 chart to be completed by hospital staff departments to document physician record of identification and referral of organ donors. Records case histories over time, including patient information, risk management reports, medical records problems, patient mortality, medication use, utilization review and top three DRGs.

Physicians are rated as follows:

A = Management was appropriate

B = Standard of care was not met

C = Substandard care with adverse outcome

D = Standard of documentation was not met

P = Pending
