Screener Survey
EPA is asking all Aquatic Animal Production Facilities to respond to
the Screener
Questionnaire (PDF, 34KB). This survey was approved on July
27, 2001, OMB Control No. 2040-0237. This survey seeks information on
production (methods) practices, species produced, production levels wastewater
treatment, and discharge and corporate structure. The information collected
through this screener will be used to support the development of the effluent
guidelines for the aquatic animal production point source category.
For those of you responding to the screener questionnaire using the downloaded
version, please include the four digit survey identification number found
on the original questionnaire addressed to you or on your reminder letter.
On an original questionnaire, this number is found on the upper right
hand corner of the page. On a reminder letter, the number is found under
your address Voluntary submissions of screener questionnaires from aquatic
animal production facilities that have not received the questionnaire
directly from EPA are also welcome. Please do not include a survey identification
number if you never received a questionnaire or reminder letter directly
from EPA.
