OSC Forms
The following forms can be filled out electronically and are available in both
HTML (web page) and PDF (Acrobat) format. It is recommended that you save
the form to your computer prior to filling it out. The PDF version must be filled
out using Adobe Acrobat
Reader which can be freely downloaded from Adobe. For the Complaint Form (OSC-11) and
Whistleblower Disclosure Form (OSC-12), please print out, sign and date the form in the areas indicated, and
then mail the completed form to the address indicated.

Notes:
1. Data entered on Web (i.e., HTML) versions
of forms currently can only be printed out in hard copy, but cannot be saved to
the hard drive of the filer's computer. For PDF versions of the OSC-11 and
OSC-12 forms all data entered can be saved for later retrieval when using
Acrobat Reader version 5.05 or higher.
2. Web (HTML) versions permit data entered into multi-line fields to scroll beyond
the initial field size. Only data displayed within the initial field area will
be viewable on printed hard copy.
