Application Forms All non-immigrant applicants applying for a visa must complete the visa application forms. The DS-156 must be completed on line, and the applicant must submit all three pages. The forms must be signed by the applicant himself. Anyone assisting the visa applicant fill out the forms must also sign the DS-156. Each application must be accompanied by a recent photograph at least 50mm x 50mm with a white background. Each applicant must also pay a nonrefundable visa application fee. Visa Application Forms:
