NON-IMMIGRANT PROCEDURES

Application Forms


All non-immigrant applicants applying for a visa must complete the visa application forms. The DS-156 must be completed on line, and the applicant must submit all three pages. The forms must be signed by the applicant himself. Anyone assisting the visa applicant fill out the forms must also sign the DS-156. Each application must be accompanied by a recent photograph at least 50mm x 50mm with a white background. Each applicant must also pay a nonrefundable visa application fee.

DS-156: Nonimmigrant Visa Application Form. ." All non-immigrant visa applicants must submit this form. The following forms can be downloaded (Adobe Acrobat version 5.0 or above required).

DS-157: Supplemental Nonimmigrant Visa Application Form. All applicants over the age of 16, regardless of nationality, must also fill out this form. The following forms can be downloaded (Adobe Acrobat version 5.0 or above required).

DS-157 (English) DS-157 (日本語訳付)

DS-158: Contact Information and Work History for Nonimmigrant Visa Applicant.   In; addition to above two forms, F, M and J student and exchange visitor visa applicants need to complete and submit a form DS-158. The following forms can be downloaded (Adobe Acrobat version 5.0 or above required)
