OCHP issued the following solicitations in FY 04:

    SUMMARY: Requirement due dates listed in the previously published solicitation: Solicitation: ‘‘Building State, Territorial, and Tribal Capacity to Address Children’s Environmental Health: Environmental Triggers of Childhood Asthma’’ have been changed. The original solicitation, [FRL–7818–5] was published in the Federal Register on Monday, September 27, 2004/69 FR 57695–57705. These changes are being made to allow eligible applicants sufficient time to learn of this funding opportunity, develop strong Letters of Intent, and, if found eligible by EPA based on the Letters of Intent, to submit a strong Full Proposal.

    Due dates under this solicitation are being changed to allow eligible applicants more time to learn of this funding opportunity, develop stronger Letters of Intent and, if found eligible by EPA based on the Letters of Intent, to submit a stronger Full Proposal.

    • Solicitation Closing Date and Full Proposal Shipping Date for applicants found to be eligible by EPA based upon the Letters of Intent: February 1, 2005
    • Selected Projects Announced: Around early summer 2005
    • Earliest Start Date to Plan On in Time Lines: July 15, 2005

Following is the EPA Grant Application Kit that is required for submission and consideration of a grant proposal. Forms are provided as WordPerfect 6/7/8/9 (209KB) documents and as a PDF file (234KB). Download the forms, print them out and manually enter the requested information. Electronic submission of the application is not available.

Include with your grant submission the Application Receipt Letter (as provided in application files above) completed with your name and address. The completed Application Receipt Letter will be stamped with your grant identification number by the U.S. EPA Office of Children's Health Protection and will be returned to you as confirmation that your proposal has been received and logged in.
